E-mail address: [email protected] ABSTRACT Coronary heart disease is the leading cause of morbidity and mortality in patients with diabetes mellitus, and causes changes in the endothelium and vascular smooth muscle. This endothelial dysfunction is a precursor of atherogenic lesions. This article describes the case of a diabetic patient with left main trunk disease who was successfully treated with percutaneous coronary intervention and showed rapid progression of atherosclerotic disease in other vessels, so she needed new percutaneous revascularization. Angiographic images are presented and aspects of endothelial dysfunction in diabetes mellitus and its percutaneous treatment are commented. It is important to early identify and treat endothelial dysfunction in diabetic patients. The choice of the revascularization method should be individualized.

EDITORIAL COMMENT Outcomes in Diabetics Undergoing Revascularization

Diabetes mellitus, a major determinant of cardiovascular events, portends an adverse prognosis in patients with coronary artery disease regardless of treatment strategy. Among patients undergoing coronary revascularization procedures, the Bypass Angioplasty Revascularization Investigation (BARI) trial (1) demonstrated an unfavorable interaction between diabetes and an initial strategy of percut...

Cost-effectiveness of percutaneous coronary intervention with drug eluting stents versus bypass surgery for patients with diabetes mellitus and multivessel coronary artery disease: results from the FREEDOM trial.

BACKGROUND Studies from the balloon angioplasty and bare metal stent eras have demonstrated that coronary artery bypass grafting (CABG) is cost-effective compared with percutaneous coronary intervention (PCI) for patients undergoing multivessel coronary revascularization-particularly among patients with complex coronary artery disease or diabetes mellitus. Whether these results apply in the dru...

Diabetic Heart Disease and Interventional Cardiology: How Can Clinical Outcomes Be Improved? Revascularization Guided by Hemodynamic Parameters

Diabetes mellitus (DM) is an increasingly common disease, with an estimated prevalence of greater than 15% worldwide. Patients suffering from DM experience an increased risk of coronary heart disease, with a concomitant raise in cardiovascular morbidity and mortality. Accordingly, these patients represent 20%-30% of those undergoing revascularization procedures worldwide. Advances in the safety...
